Insurance Accepted

Dr. Krzykowski accepts the following insurances: Blue Cross Blue Shield, Aetna, Midland Choice, and United. Additionally, she is on panel for some Cigna plans. Even if you have one of the listed plans, please contact your insurance directly via the number on the back of your insurance card to ask if Dr. Krzykowski is covered under your plan prior to your first appointment.

She is a preferred provider for Methodist and is a Tier 2 provider for UNMC.

For those who do not have or do not wish to use their insurance, Dr. Krzykowski has a cash pay rate of $175.00 per hour, after the initial appointment cash rate of $225.00. She does not offer sliding scale rates.

The practice will bill your insurance company for you. You are responsible for any amounts not covered by insurance including deductible amounts and copays. If you are unsure what your out-of-pocket amount will be, you can call the number on the back of your insurance card to determine that amount.
